STEPS TO REPRODUCE:
1. Go to Service Level Management Console.
2. Open Service Target SLM74123.
3. Go to the Milestones Tab.
4. Click on View Button to View Milestones Actions

ACTUAL RESULT:
Getting Error - The value(s) for this entry violate a unique index that has been defined for this form (ARERR 382)
From SLM Console if we tried to Build the service target the get error for Unique Index Violation
and Stuck in Could not Build Status

EXPECTED RESULT:

The error should not occur and SVT should be build successfully

The defects is targeted to fix on IT Service Management Suite 21.3.x and 22.x

As a fix, the below steps can be performed.
  • Go to the Midtier.
  • Open the form , SLM:RuleActionSetValueItem.
  • Search with Record Status ="Clone".
  • Delete all the records..
  • Import the Active Link provided in the Article.
